Pillows: Loft, Fill and Choosing by Sleep Position

Pillows arranged on a made bed with white bedding

People spend months choosing a mattress and then pick a pillow off a shelf because it felt soft in the packaging. The pillow is what holds your head in line with your spine all night.

And unlike a mattress, the right pillow is determined almost entirely by one thing: the position you sleep in.

Here is how to match loft and fill to how you actually sleep.

Loft: The Specification That Matters

Loft is the height of the pillow when it is fluffed and lying on the bed, before you put your head on it. It is the single most important pillow measurement.

You sleep mostly on your…Loft neededFirmnessWhy
Side4–5″ (high)Medium-firm to firmFills the gap between shoulder and head
Back3–4″ (medium)MediumSupports the neck curve without pushing the head forward
Front2–3″ (low)SoftA thick pillow forces the neck backward
Combination3–4″, adjustableMediumShredded fill lets you tune it

The aim in every case is the same: your head, neck and spine in a straight line. Side sleepers need the most loft because the shoulder creates the largest gap to fill.

Broader shoulders need more loft than the table suggests. If you are broad-shouldered and sleep on your side, work from the top of the 4 to 5 inch range rather than the bottom.

The mirror test

Lie on your side on a bed with the pillow you are considering and have someone look at you from the front, or use a phone camera.

Your nose should line up with the centre of your chest. Chin tucked toward your chest means the pillow is too high; head tilted back means it is too low.

Fill Materials

DownMinimalWarm2–5 yearsLuxurious feel; flattens and needs fluffing
Down alternativeMinimalWarm1–2 yearsCheapest; flattens fastest

Shredded memory foam is the most forgiving choice if you are unsure. Being able to remove fill until the loft is right effectively lets you correct a buying mistake at home.

Down and down alternative feel wonderful and support very little. They suit front sleepers, who need almost no loft, and disappoint most side sleepers within a week.

Sizes

Pillow sizeDimensionsSuits
Standard20″ × 26″Twin and full beds; most sleepers
Queen20″ × 30″Queen beds; anyone who moves at night
King20″ × 36″King beds; two across the width
Body pillow20″ × 54″Side sleepers; pregnancy support

Two standard pillows do not span a king bed, and two kings overhang a queen. Match the pillow size to the bed and the pillowcases become straightforward too — our mattress size chart covers the bed dimensions.

When to Replace

Pillows wear out far faster than mattresses and are replaced far less often than they should be.

  • The fold test — fold the pillow in half; if it stays folded rather than springing back, it is finished
  • Permanent lumps, flat patches or a visible dip where your head sits
  • Waking with neck stiffness that eases during the day
  • Persistent odour that washing does not remove
  • Most synthetic pillows last one to two years; foam and latex rather longer

Neck pain has many causes and a pillow is only one of them. Guidance from OrthoInfo, published by the American Academy of Orthopaedic Surgeons, is a sensible reference, and persistent pain is worth raising with a doctor rather than solving through bedding alone.

Protecting What You Buy

A pillow protector under the pillowcase does the same job a mattress protector does, for a fraction of the cost.

  • Keeps moisture and skin oils out of the fill, which is what breaks pillows down
  • A tightly woven protector creates a barrier at the sleep surface for allergen control
  • Far easier to wash weekly than the pillow itself
  • Foam pillows in particular should never go in a washing machine — the protector takes that burden

Our guide to mattress protectors, pads and toppers covers the same logic applied to the mattress underneath.

Specialist Pillows

Beyond the standard rectangle, a few shapes solve specific problems — and a few are marketed far beyond what they deliver.

Body pillowSide sleepers; hip and knee alignment; pregnancyTakes considerable space on a smaller bed
WedgeReading in bed; elevating the upper bodyAn adjustable base does this better long term
Cooling / gel-infusedHot sleepers, modestlyAirflow and fill type matter more than the gel

Body pillows are the most consistently useful of these for side sleepers, because supporting the top leg stops the hips rotating out of alignment during the night.

If you are using a wedge pillow regularly to sit up, an adjustable bed base does the same job without sliding down the bed.

Frequently Asked Questions

What pillow loft do side sleepers need?

Around 4 to 5 inches, in a medium-firm to firm pillow. The shoulder creates a wide gap between the mattress and your head, and the pillow has to fill it to keep the neck straight.

How do I know if my pillow is too high?

Lie on your side and check whether your chin is tucked toward your chest. If it is, the pillow is too high. Head tilted back means it is too low. Your nose should line up with your sternum.

How often should I replace a pillow?

One to two years for synthetic fills, and three to five for latex or quality foam. Fold it in half — if it does not spring back open, it has stopped supporting you.

What is the best pillow fill?

Shredded memory foam is the most adaptable, because you can remove fill until the loft suits you. Latex lasts longest and sleeps coolest; down feels the most luxurious and supports the least.

Do I need a king pillow for a king bed?

Not necessarily, but two standard pillows leave a gap across a 76-inch bed. Two king pillows span it properly, and the bedding looks intentional rather than undersized.

Try It Lying Down

A pillow squeezed in your hands tells you nothing. Lying on one in your usual sleeping position for a minute tells you everything.

Our mattress range in Newport News includes pillows and protectors, and you are welcome to test a pillow on a display mattress rather than guessing at the loft.

The mattress underneath changes what loft you need — a soft mattress lets your shoulder sink, reducing the gap a side sleeper has to fill. Our comparison of memory foam, innerspring and hybrid construction covers that.
